Grievance Policy

1.  In-order for any Laser Intern Clinician, who has officially enrolled in any course at MALE, has paid the tuition and/or other fees, has attended the percentage of hours state below, and request to be withdrawn from the program, must officially withdraw with the Director of Operations by completing and submitting an Official Withdrawal Request Form. 

2.  If a Laser Intern Clinician requests to withdraw prior to attending the first day of class; they must officially withdraw with the Director of Operations by one of three ways; email (with confirmation of receipt), certified mail (with signature of acceptance), or in person by appointment only.

3.  The postmark on written notification, or the date the information is delivered in person to the MALE Enrollment Office, will determine cancellation dates.

4.  In the event MALE cancels a program, the Laser Intern Clinician will receive a 100% refund. 

5.  Refunds will be determined, less any non-refundable fees. 

a.  A Laser Intern Clinician "no-show" for the first day of class or never attending class is not an automatic refund. 

b.  A “stop payment” on a check, or a filing a dispute on a credit card issued to MALE may result in additional fees and a report filed with the Maricopa County Attorney’s Office. 

c.  The refund policy stands regardless of payment method. (i.e., if an agency elects not to pay fees and tuition due for a Laser Intern Clinician who drops/withdraws or is withdrawn for violating school policy, the Laser Intern Clinician is responsible for payment).

d.  The Laser Intern Clinician may submit a Refund Appeal Request Form, with supporting documentation, to the Director of Operations and received before the last day of the schedule class time frame for which the withdrawal took place. 

e.  Laser Intern Clinicians are responsible to ensure their mailing address and contact information is up to date, at all times, as any refund checks will be sent certified mailed to the Laser Intern Clinician’s address of record with in 7 business days of receiving the Official Withdrawal Request Form.

Grievance Policy

A Laser Intern Clinician who feels that he/she has been treated unfairly or unjustly by an employed instructor of the institute regarding an academic process such as grading, testing or assignments, should discuss the issue first with the instructor involved.  This conference should be requested immediately from the time the Laser Intern Clinician knew or reasonably should have known about the unfair or unjust treatment. 

This instructional grievance process should not be utilized in a case in which a Laser Intern Clinician feels he/she has experienced discrimination. If the Laser Intern Clinician feels that he/she has experienced discrimination based on race, color, religion, national origin, gender, age, disability, veteran status, or sexual orientation, the Laser Intern Clinician should refer to the Director of Operations for guidance. 

Steps for Laser Intern Clinician Clinicians to follow for Academic Grievance: 

1.  A request for conference with the instructor. If the problem is not resolved or the instructor has been unable to meet with the Laser Intern Clinician, the Laser Intern Clinician may continue the process by filing a written complaint with the Director of Operations. 

2.  This written complaint must be filed within three business days from the initial incident with specific documentation including dates, times, materials, etc.

3.  The written complaint will be made available to the instructor and an official meeting with the Director of Operations will work with the parties to resolve the conflict. 

4.  Every attempt will be made to maintain confidentiality during this process.

5.  The instructor will not be required to respond to a complaint which is not in writing and which, when appropriate, that is not have specific documentation including dates, times, materials, etc. 

6.  If the grievance is not resolved at this level within three (3) business days from the date of the official meeting, the Laser Intern Clinician should forward to the original written complaint to the Director of Operations, Isabel Calleros, along with an explanation regarding action taken at each prior level. 

7.  A second meeting will be scheduled with the Laser Intern Clinician, Director of Operations, the instructor, and the Medical Director to resolve the issues. 

8.  If after the grievance is not resolved with the Medical Director, the Laser Intern Clinician has the option to send the written grievance to the Arizona Department of Health Services (ADHS) Telephone: 602-542-1025 | Address: 150 North 18th Ave., Phoenix AZ 85007.

9.  ADHS will issue a final written determination in the grievance process as the final step of this process.

CHEATING POLICY

Arizona Department of Health Services (ADHS) provides the opportunity to the training institute to proxy the state approved final examination to the Laser Intern Clinician.  Each Laser Intern Clinician, taking any of the courses in this program are expected to abide by the “NO CHEATING POLICY”.  Any work submitted by a Laser Intern Clinician for academic credit shall be the Laser Intern Clinician's own work unless a group project is assigned. 

TERMINATION

MALE provides an environment conducive to learning.  Any Laser Intern Clinician found disrupting the learning environment by demonstrating unacceptable conduct, use of foul language, disrespect to fellow students, staff, patients or faculty, will be terminated immediately.  The refund policy will apply.  A written letter of termination will be given to the Laser Intern Clinician either in person or through certified mail within 24 hours. 

APPEAL PROCESS

Any Laser Intern Clinician who wishes to re-enter classes may appeal any decision made by MALE.  A letter of appeal should identify the circumstances for termination and any rebuttal the Laser Intern Clinician wishes to clarify.  The board of advisors will meet within 24 hours to review all information provided.  All appeals decisions are final.  If the decision is for the return of the Laser Intern Clinician, he or she may resume classes on the next following scheduled class.

ADMISSION

MALE has set the standards of professionalism and excellence in the education of ParaMedical & Cosmetic lasers.  Strict enrollment requirements and graduating skills separate our Laser Intern Clinicians from the standard students.  Our class enrollments are limited to five students, per class, in order to provide one-on-one close instruction and full laser applications per patient.  

PREREQUISITES:

Every state has different local and state laws that govern laser technicians and laser services.  MALE is governed by the Arizona Department of Health Services.  At this time there are no prerequisites required to enroll in the ParaMedical or Cosmetic Laser Program. If not in the state of Arizona, check with the state in which they choose to practice for clarification on state laws to perform laser treatments and/or reciprocity. In the state of Arizona there are no prerequisites to become a Certified Laser Technician.

ENROLLMENT REQUIREMENTS:

To enroll in any of the MALE Laser courses, the Laser Intern Clinician must comply with the following:

1.  Interview with the Director of Operations (via a live onsite tour, Zoom or telephone).

2.  If applicable, although not required for the state of Arizona, present the following documentation:

a.  Valid professional license (example: aesthetics, cosmetologist, or medical from any state)*

b.  If practicing in another state, check with state and local agencies for professional requirements.  

3.  Proof of identification from any state or country (present only one of the following): 

a.  Any valid state ID or driver’s license, Passport, Military ID or Resident card

4.  Two passport photos

5.  Must be at least 18 years of age

6.  Complete and sign the MALE Enrollment Contract and other enrollment documents

7.  If applicable, present any approved and accepted scholarships or grants 

8.  Pay course fees according to MALE Enrollment Contract

STUDENTS WITH DISABILITIES

In compliance with MALE’s disabilities policy and equal access laws, we are available to discuss appropriate academic accommodations that may be required for prospective Laser Intern Clinician with disabilities. Requests for academic accommodations are to be provided, in writing, two weeks prior to commencing the course so arrangements can be made. 

LEAVE OF ABSENCE

The Leave of Absence (LOA) is designed for a short time away from laser studies. 

·  LOA is issued for the maximum of six (6) months (180 days).

·  When a course cannot be completed in its entirety, a LOA is submitted in writing to the Director of Operations via email or as a written signed document.  

·  The LOA begins from the date of the signed approval for the same course.

·  The Laser Intern Clinician will be notified by email the acceptance or declining of the requested LOA.  

·  There are no monetary refunds for a LOA. 

RE-ENTRY REQUIREMENTS

When a Laser Intern Clinician chooses to re-entry the course, a written request is submitted via email or as a written signed document and must be presented to the Director of Operations as to the schedule class date they desire to return.  The course must be taken in its entirety.  All late fees must be paid prior to re-entry.

JOB PLACEMENT

MALE has included in the program courses and classes the importance of employment experience through the MALE Internship Program.  We strive to provide our students the knowledge of laser skills, customer service and skin anatomy in preparation for a successful career as a laser technician. 

MALE will assist Clinical Laser Interns with job placement referrals.  MALE has affiliation with local med spas and medical practices that from time-to-time call for laser technician placement.  Under government regulations, this training facility  cannot guarantee job placement to any students. We do assist you with all possible employment leads that flow through our office, letters of accomplishments and referrals.
